Provide direct patient care as a Clinical Pharmacist I at AHS, focusing on medication therapy management. Collaborate with healthcare teams to ensure protected and effective medication use. In this full-time position at the U of A Stollery Children’s Hospital, you will engage in patient care, assessing drug-related problems and formulating treatment plans.
Responsibilities include maintaining patient health records and participating in quality improvement initiatives. A significant part of your role will also be participating in patient care rounds as applicable. Key Responsibilities:
- Manage medication therapy in a patient-centered model
- Assess and resolve drug-related issues for patients
- Participate in patient care rounds with healthcare teams
- Document patient care activities clearly in health records
- Oversee preparation and distribution of medications
Requirements:
- Bachelor of Science in Pharmacy
- Registration with Alberta College of Pharmacy as a Pharmacist
- Experience with medication systems preferred
- Ability to communicate effectively in English
- Proven problem-solving and time management skills
