JOB SUMMARY:

Within the context of a patient, client and family centred model of care and, in accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff.

Reporting to the Patient Care Manager, Centre for Healthy Aging, the Clinical Resource Nurse, Older Adults functions as an integral member of the multidisciplinary team, collaborating on the plan of care for ambulatory patients/clients referred to the Older Adults Clinic. Position utilizes evidence-informed practice and advanced theory and knowledge of content area to provide consultation and clinical expertise to the care team, educators and students related to care for older adults. Supports the adoption and implementation of a Learning Health System (LHS) as a strategy to improve health system performance, enhance care delivery and respond to the evolving needs of Older Adults in alignment with cutting-edge research evidence and resource stewardship.

Collects and monitors program data and evaluates the effectiveness and appropriateness of educational programs, activities and resources. Implements and evaluates training programs using advanced teaching and learning principles. Develops and revises training and documentation as needed.

Consults and collaborates with team members/other caregivers in the identification and resolution of client/family care issues. Participates in research, policy, quality improvement, and risk management activities.

Role models direct care skills to staff and students, performs nursing assessments and ongoing evaluation of older adults with complex care needs. Provides on-going assessment and care management and develops and initiates care plans for clients anticipating or experiencing episodic acute illness when needed. Accesses resources to support discharge planning. May be required to work at multiple sites across Providence Health Care.

Participates in nursing leadership activities locally, nationally, and internationally to influence geriatric/older adult practice change and to meet the strategic priorities of PHC's Older Adult Program. Liaises and communicates with a range of health care professionals and other resources by leading and participating in a variety of regional and local committees and/or research projects to represent older adult care.

QUALIFICATIONS:

Education, Training, and Experience

Current practicing licensure as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM) plus five (5) years’ recent, related experience including (3) three years’ in gerontological nursing and one (1) year of leadership/supervisory experience or an equivalent combination of education, training and experience.

Certification in a geriatric specialty program or completion of post graduate course work and/or certification applicable to gerontological nursing, such as the Canadian Nurses Association Gerontology Nursing Certificate, would be an asset.

- Uses advanced clinical judgment and nursing knowledge to complete standardized comprehensive patient/client assessments and develop patient-specific care plans in collaboration with the inter-professional team. Arranges referrals to Geriatricians and various Allied Health services, and coordinates referrals and linkages to appropriate community resources, including Older Adult programs such as specialized senior’s clinics.
- Works with patients/clients/families with dementia to assist them with the management and treatment of specific medical conditions, their symptoms and self/home care skills utilizing principles of teaching and learning. Provides relevant information to clients/family, significant others, health team members and the community to assist the client/family in making informed decisions. Advocates for and facilitates informed choices.
- Provides direct care to a wide spectrum of aging clients and their families by receiving referrals, providing a comprehensive nursing assessment, prioritizing care requirements, ensuring testing is complete, assessing and interpreting results within their scope of practice and individual competence, and monitoring client’s progress according to expected outcomes. Establishes therapeutic relationships with patient/clients and families and provides emotional support by identifying caregiver stresses, clients' and families' perspectives, learning needs and readiness and ability to learn.
- Uses teaching and learning principles and a program planning and evaluation process to develop, implement and evaluate staff education programs. Conducts formal/informal presentations/in-services and demonstrations and facilitates groups. Coaches and mentors staff to introduce new skills/procedures for implementing effective methods for triaging the designated population group. Arranges and/or provides preceptorships and mentorships for and students regarding older adult health care, as appropriate.
- Collects and maintains required statistics and data related to the intake/referral process to facilitate triage and waitlist management. Conducts data analyses to determine trends and assist with forecasting and planning. Prepares reports and other documentation as requested by the Manager, distributes statistics and reports to members of the care and management team as required.
- Works collaboratively to coordinates a network of relationships with community care agencies/services to address care needs of clients and facilitate follow up care, arranging access to a variety of specialized services and clinics. Arranges for access to other resources such as respite beds, additional home support services and home care. Ensures care and service plans with clients and families/care givers are appropriate and understood.
- Functions as an expert clinical resource to nursing staff and other health care clinicians by providing consultative services, advice and education sessions together within the clinic and throughout PHC on issues related to the promotion of standards of care of the older adult, reviewing client/family needs or a specific client's progress, problem solving and providing guidance/knowledge in the management of the applicable client or client group. Acts as a role model while coaching team members and in using communication skills to effectively interact with clients and family members.
- Demonstrates expertise and provides leadership in evidence-based nursing practice by reviewing literature, consulting with clinical and community experts, analyzing outcome data, evaluating clinical practice and referral systems, identifying trends and opportunities in geriatric triage,



developing new and/or revised standards and developing and presenting education sessions based upon specialized knowledge.
- Participates in interdisciplinary team activities required to enhance and optimize the delivery of care and to facilitate evidence-based nursing practice. Coordinates and participates in quality assurance and risk management activities for assigned area by evaluating effect of changes in client care practice and recommending alternative approaches and changes to selected policies, procedures or protocols.
- Promotes the principles of a Learning Health System (LHS) to advocate for the integration of multiple dimensions of knowledge, practice, and experience, fusing elements such as research, knowledge synthesis, implementation support, patient/client partner engagement and co-design, quality improvement, data analysis, and evaluation, as an approach to bridge the gap between health research evidence generated and its use in clinical care.
- Participates in relevant research and quality improvement projects by assisting the Manager, the Professional Practice Director/Leader and other health care professionals by summarizing designated information and/or providing verbal updates for review and further analysis by principal researcher, research team and/or project coordinator.
- Facilitates the process of preserving and enhancing clients’ ability to live independently, utilizing crisis intervention and individual and family counseling according to the approved plan of treatment. Consults with physician regarding physical and medical issues/needs. Reorganizes priorities in response to constantly fluctuating workload volumes and client treatment requirements.
- Attends a variety of regional and local committees to facilitate the resolution of issues, provide input and act as an advocate in the achievement of the goals and objectives of the program and to promote the optimal support of the older adult client group throughout the continuum of care.
- Participates in nursing leadership activities locally, nationally, and internationally to influence older adult practice change and to meet the strategic priorities of PHC's Older Adult Program. Participates in clinical care, systems leadership, advancement of nursing practice, and evaluation and research as the needs of the Older Adult evolve.
- Maintains complete and accurate computerized and paper records by inputting data and documentation in accordance with established procedures and policies.
- Responds to incidents such as missing clients, injuries to staff or visitors, Code Blue/White emergencies, hazardous spills, fire and aggressive/disruptive behaviour to ensure medical and other support is accessed in a timely manner. Informs designated staff of occurrences and completes Incident Report forms in accordance with established procedures and policies.
