Responsibilities

- Conduct client intake by reviewing background information, interviewing clients, gathering relevant case histories, and preparing accurate intake documentation.
- Assess clients’ strengths, abilities, needs, challenges, and available supports to determine appropriate services and interventions.
- Support clients in identifying available options, establishing realistic goals, and developing individualized plans of action based on their needs.
- Assist clients in determining their eligibility for government advantages, social assistance, and other community support programs.
- Connect clients with appropriate community resources and social services, including housing, healthcare, legal assistance, financial supports, employment services, transportation, childcare, and other essential services.
- Provide guidance and ongoing support to clients living in group homes, transitional housing, or other supported living environments, including assistance with transition and community reintegration planning where applicable.
- Participate in client screening, intake, selection, and admission into appropriate Daveli Community Services programs and services.
- Assist with the delivery of life-skills training, behaviour-management initiatives, substance-use support programs, youth programs, and other community-based services under the direction of appropriate social service or healthcare professionals.
- Conduct regular follow-ups with clients to review their progress, identify emerging concerns, provide support, and address challenges affecting their established goals.




- Monitor and document clients’ progress and responses to interventions to support the evaluation and improvement of programs and services.
- Provide information and assistance to individuals accessing social assistance, pensions, income-support programs, and other available benefits.
- Provide appropriate support during crisis situations and connect individuals and families with emergency shelters, crisis services, and other immediate community resources.
- Coordinate and support the delivery of designated community programs, activities, workshops, and support services offered by Daveli Community Services.
- Establish and maintain professional working relationships with community organizations, social service agencies, healthcare providers, and other professionals involved in clients’ care and support.
- Exchange relevant information with authorized service providers, while maintaining confidentiality, to promote continuity of services and monitor clients’ overall progress.
- Recruit, organize, coordinate, and support volunteers involved in Daveli Community Services programs, community activities, and outreach initiatives.
- Maintain accurate and confidential client records, case notes, service statistics, referral documentation, program participation records, and other information required for program evaluation and reporting.
- Assist with evaluating the effectiveness of community programs by monitoring outcomes, identifying service gaps, and providing feedback for continuous program improvement.
- Provide guidance and task coordination to social service support workers and volunteers when assigned.
- Perform other related community and social service duties consistent with the programs and objectives of Daveli Community Services.

Requirements

- Completion of a college diploma or university degree in Social Service Work, Community and Social Services, Psychology, Sociology, Child and Youth Care, Social Work, or a related social science/health field.




- Previous experience working in community services, social services, healthcare, settlement services, supportive housing, youth services, or other human-service settings is an asset.
- Knowledge of community resources, including housing, employment, healthcare, financial assistance, legal services, transportation, childcare, and social assistance programs.
- Ability to assess clients’ needs, identify strengths and barriers, develop appropriate service plans, and make community referrals.
- Strong interpersonal, communication, counselling, problem-solving, and crisis-intervention skills.
- Ability to work effectively with individuals and families from diverse cultural and socioeconomic backgrounds.
- Strong organizational and time-management skills with the ability to maintain accurate client records, case notes, program statistics, and reports.
- Ability to maintain professional boundaries, confidentiality, and client privacy.
- Ability to work independently and collaboratively with healthcare professionals, social service agencies, community organizations, volunteers, and other service providers.
- Proficiency with computers and standard office applications for documentation, communication, and reporting.
- First Aid and CPR certification is an asset or may be required depending on program responsibilities.
- Vulnerable Sector Check/Criminal Record Check may be required before employment, depending on the client population served.
- Ability to work flexible hours, including occasional evenings or weekends, based on program and client needs.
- A valid Ontario driver's licence and access to reliable transportation may be considered an asset where community outreach or client-related travel is required.

Pay: $36.60-$38.00 per hour
