Company Description Brain Changes Initiative (BCI) is a not-for-profit organization dedicated to funding innovative research that improves the standard of care for Traumatic Brain Injury (TBI) patients. BCI’s mission centers on developing a standardized Six Modalities of Brain Health program that focuses on intensive aerobic and cognitive exercise, nutrition, mindfulness, sleep hygiene, and limiting harmful exposures to support optimal recovery. The organization works to expand access to care by advocating for changes in clinical protocols and translating knowledge directly to TBI survivors and their families through information, support, and services.

Role Description The Implementation Coordinator is a full time, hybrid role based in Scarborough, ON, with some flexibility for work from home. This position is responsible for research and implementation of the Six Modalities of Brain Health program at the East GTA Family Health Team.

The Implementation Coordinator will be engaged in the following activities:

- In consultation with the Clinical Lead, using the research evidence (recently completed scoping reviews on the Six Modalities of Brain Health and the Canadian TBI Guideline) will develop a manualized multi-disciplinary Primary Care Health Team model encompassing the 6 lifestyle modalities proposed by Brain Changes Initiative
- In consultation with the Clinical Lead will create the clinical implementation protocol that all patients enrolled in this novel model will receive
- In consultation with the Clinical Lead will identify and develop mechanisms to collect structure, process and outcome data quality indicators
- Will prepare necessary Research Ethics Board documents to obtain approval for this Quality Improvement project
- Will enroll patients into the novel care model,



ensuring informed consent and all implementation practices are consistent with the approved quality improvement implementation practices
- Will ensure consistent implementation of the clinical, data collection and research protocols and oversee the implementation activities of the clinical team
- Will conduct data analysis to evaluate the clinical and implementation outcomes
- Will prepare an implementation report and a peer-reviewed manuscript
- Will ensure transparency in all activities and will document all activities to allow for future scale and spread of any/all aspects of the model that show clinical efficacy
- Will supervise and delineate duties of any/all trainees involved in the project
- Will write any grant application(s) needed to support this project

This novel model of care will be implemented at the Scarborough East GTA FHT at 520 Ellesmere Rd, Scarborough ON and attendance at this site will be required at least 2 days per week.

Qualifications

- Candidates should possess strong Analytical Skills to interpret data, monitor program performance, and support evidence-based decision-making.
- Candidates should possess solid Communication skills to collaborate with multidisciplinary teams, engage stakeholders, and prepare clear reports and updates.
- Candidates should possess a focus on Patient Satisfaction to support TBI survivors, families, and partners with empathy, responsiveness, and professionalism.
- Candidates should possess Project Management skills to coordinate timelines, resources, and tasks across multiple initiatives and ensure successful implementation.
- Additional beneficial qualifications include experience in healthcare or non-profit environments, familiarity with neuro-recovery or rehabilitation programs, proficiency with productivity and project management tools, and strong organizational and time-management abilities.
- Post-secondary education in health sciences, social sciences, research, or a related field, or equivalent combination of education and experience, is preferred.
