The Intake Coordinator will be responsible for maintaining front-office client intake, which includes conducting initial interviews with new adult clients and coordinating with the clinical team to ensure clients are registered in programs that best support their needs.ResponsibilitiesConducts initial interviews in person or over the phone with current clients to gather information related to their current end-of-life experience, mental health, supports, and perceived needs,



and to provide appropriate information related to hospice programsWorks with clinical team to register clients in appropriate programsEnters and maintains client files in databaseCollaborates with other staff in the development of client programsProvides other administrative support to the clinical team as neededAttends all related meetings including Intake Review MeetingsCovers reception as neededPerforms other duties as requiredQualificationsTwo years of post-secondary education in a related fieldTwo or more years of experience as an administrative office assistant is preferredApplicable knowledge and understanding of the philosophy of Hospice palliative care and the needs of the dying, their families, and the bereavedExcellent communication, organizational, and time management skillsProficiency in Microsoft Office Suite and ability to learn new systemsAbility to communicate compassionately and professionally with people from diverse backgroundsAdditional CriteriaCriminal Record Check requiredVehicle and valid driver’s license requiredCurrent CPR/First Aid certification is considered an asset#J-18808-Ljbffr
