Job Summary

Duties
- Deliver comprehensive patient care by monitoring vital signs, administering medications, and assisting with daily activities.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient’s needs.
- Assist with procedures such as phlebotomy (blood draws), wound care, and pain management strategies.
- Maintain accurate medical documentation using electronic health record systems like Epic and Cerner, ensuring compliance with healthcare regulations.
- Support dementia, senior, hospice, and pediatric patients by providing specialized care that promotes comfort and dignity.
- Educate patients and families on treatment plans, medication administration, and health maintenance strategies.
- Ensure safety protocols are followed diligently during patient interactions and emergency situations.
Experience
- Prior experience working in nursing homes, hospitals, or assisted living facilities demonstrating strong patient care skills.
- Hands-on experience with acute care procedures including medication administration, laboratory testing, and pain management.
- Familiarity with electronic health records systems such as Epic or Cerner is highly desirable.
- Background in home care or hospice environments showcasing adaptability and compassionate support for diverse patient populations.
- Knowledge of medical terminology combined with laboratory experience and case management skills to coordinate comprehensive care plans.
- Experience working with children or seniors in specialized settings like pediatrics or senior care adds valuable insight.
- Proven ability to handle multiple responsibilities efficiently while maintaining a positive attitude in fast-paced healthcare settings.
