Scope:

Under the direction of the Nurse Coordinator/designate, the LPN assess, plans, implements and evaluates nursing care for their work assignment in accordance with competency guidelines within the standards of practice as outlined by the College of LPNs of BC and current legislative scope of practice.

Key Responsibilities:

- Responsibilities include working with the care team to assess, plan, implement and monitor residents safety.
- Performs nursing procedures such as applying and changing sterile and/or non-sterile dressings, colostomy care and irrigation, taking/recording blood pressures and/or rectal temperature, administering suppositories and enemas, taking temperature, pulse and/or respiration. Administers medication as required and in compliance with practice guidelines.
- Transcribes and processes Doctors’ orders.
- Evaluates resident outcomes; records observations and reports problems and/or changes to Nurse coordinator / designate.
- Provides personal care and assistance to residents with bathing, skin care, grooming, dressing and toiletting. Encourages and assists with opportunities for recreation, spiritual and social activities.
- Feeds, assists or prompts residents to eat and take adequate fluids as indicated by care plans/worksheets. This may include the completion of tray requisitions, serving and collection of meals, and provision of food supplements and nourishment, within scope of practice.
- In conjunction with interdisciplinary team members, participates in the development of individual Resident Care Plan by identification of residents’ strengths, limitations and needs.



Contributes to discussion and selection of nursing interventions.
- Documents and reports observed changes. Completes documents and reports such as Incident Reports, Infection Reports, and Resident Assessments as required.
- Participates in admissions, discharges and transfers.
- Transfers and transports residents utilizing established techniques and mechanical aids such as wheel chairs and lifts. Accompanies residents within the facility and during outings and social activities to provide assistance as required.
- Responds to incidents/emergencies in accordance with policy/procedure and training.
- Reports hazards, unusual occurrences, accidents, and unsafe or faulty equipment/furniture to Nurse Coordinator/ designate.
- Attends in-service training and workshops as required by the employer.
- Supports the NVS Model of Service, Mission, Vision, Values and Person Centered Care Principles.
- Performs other related duties as assigned.

Qualifications

- Graduate of a recognized Practical Nursing Program
- Practising member of the College of Licensed Practical Nurses of BC, holding a valid BC Practical Nurse license in good standing
- Minimum two years of experience working with older adults
