As a Case Manager, you will provide individualized, participant-centered care, helping youth navigate housing stability and personal well-being. Your expertise in trauma-informed approaches will be essential as you manage caseloads, lead community resource education, and offer guidance on self-efficacy. Flexibility is required for early morning and evening support, as well as participation in on-call rotations.
Key Responsibilities:
• Manage caseload of 8 youth with scheduled meetings • Assist youth in creating and achieving individualized goals • Complete intakes, case notes, and reports promptly • Provide community resource information to clients • Teach skills to enhance self-efficacy
Requirements: • Degree or diploma in Human Services • 3 years experience in youth case management • Current Police and Vulnerable Sector Search • Valid Driver’s license with insurance • Suicide Intervention Awareness Training
