Location:Hybrid (Markham, Stouffville, Thornhill)Reports to:Chief Executive OfficerCompensation:$85.000About Evergreen HospiceEvergreen Hospice is a community-based, non‑profit organization providing compassionate support to individuals and families impacted by life‑threatening illness, caregiving, and grief.As the organization continues to grow—including the development of the region’s first hospice residence—Evergreen is strengthening its internal operations to support sustainability, quality care, and long‑term impact across the communities it serves.The OpportunityThis is a senior leadership role responsible for both the delivery of care and the coordination of operations that support that care.Evergreen Hospice is a growing organization where programs, people, and processes have evolved organically. This role will bring structure, clarity, and alignment across both client services and internal operations, ensuring that care is consistent, coordinated, and sustainable.Working closely with the CEO, the Director of Care will lead across counselling, intake, and program delivery—while also strengthening the systems and processes that support day‑to‑day operations.Core Areas of ResponsibilityCare & Program LeadershipLead and support counselling, intake, and community‑based programsEnsure consistent, high‑quality care across all servicesStrengthen coordination between individual, group, and volunteer‑supported careSupport development of a more structured and integrated care modelClient Care Coordination & Service DeliveryOversee intake, triage, and client flowEnsure appropriate service matching and coordinationImprove visibility into caseloads and service deliverySupport development of a clear client care coordination approachTeam Leadership & Clinical AlignmentProvide leadership and support to counselling staff and program team membersSupport supervision structures and role clarityFoster alignment, accountability,



and collaboration across the care teamOperational Coordination & SystemsStrengthen coordination across scheduling, staffing, and program deliveryImprove visibility across calendars, caseloads, and servicesOversee use of systems (e.G., AlayaCare, SharePoint) to support care deliveryIntroduce simple, practical processes to improve day‑to‑day operationsQuality, Documentation & AccreditationEnsure alignment between care delivery, documentation, and policySupport integration of accreditation standards into daily workImprove consistency in documentation practicesIntroduce light tracking to support quality and complianceVolunteers & Student IntegrationWork closely with the Manager of Volunteers to integrate volunteers and students into care deliveryEnsure clear roles, expectations, and supervision structuresSupport a positive and structured learning environmentOrganizational Growth & ReadinessSupport alignment of current programs with future hospice residence needsContribute to development of care models and service delivery structuresEnsure systems and processes can scale with growthWhat Success Looks Like (First 6–12 Months)Explicit coordination across counselling, intake, and programsImproved visibility into caseloads and service deliveryMore consistent client experience across servicesStronger alignment between care delivery and documentationImproved day‑to‑day operational flow (scheduling, communication, systems)Better integration of volunteers and students into careIdeal Candidate Profile5–8+ years in hospice, palliative care, counselling, or community healthExperience leading or coordinating care teamsStrong understanding of client‑centered care modelsComfortable working across both care delivery and operationsAble to bring structure without overcomplicating systemsCalm, practical, and solutions‑orientedPreferred:Clinical designation(MSW, RSW, RN, RP)Experience in clinical care settingsExperience working with students or multidisciplinary teams#J-18808-Ljbffr
