Position SummaryJob Title: Registered Practical Nurse *REPOST*; Status: Regular Part Time; Department: LG LTC 6W RESIDENTIAL; Program: Long Term Care; Schedule: 8‑hour shifts – days, evenings, nights, weekends (subject to change); Location: Lakeridge Health Ajax, ON – Long Term Care; Wage: $36.95–$39.02 per hour.Reporting to Nursing Leadership, the Registered Practical Nurse will provide excellent nursing care to residents at the Lakeridge Gardens 320‑bed long term care home being built on the Ajax‑Pickering hospital site. This new home is part of the government's plan to improve long‑term care delivery and capacity in Ontario and the Durham Region. The state‑of‑the‑art long‑term care home sets a current bar for seniors' care, with dining and outdoor space on every floor, a large courtyard, onsite diabetes care and other innovative features.Duties And ResponsibilitiesAssess, plan, implement, and evaluate high‑quality nursing care to residents.Assist in developing and maintaining appropriate individual care plans, ensuring they are initiated collaboratively with multidisciplinary team members.Communicate with relatives regarding resident care and condition as required.Participate in discharge planning, consultation, and/or transfer of residents.Provide direct resident care and participate in resident teaching as required.Encourage active participation by residents in therapy programs and recreational activities.Provide first aid to staff when necessary and complete Employee Injury Reports.Procure and administer medications, including injections and narcotics.Participate in orientation, on‑the‑job training,



and in‑servicing of nursing staff as required.QualificationsCurrent Certificate of Registration in good standing with the College of Nurses of Ontario as a Registered Practical Nurse.Related experience in a geriatric or Long Term Care environment is preferred.Demonstrated commitment to maintain Reflective Practice Standards as outlined by the CNO.Knowledge of and compliance with safe work practices.Strong written and oral communication, critical thinking, care coordination, time management, and customer focus skills.Empathy, concern, and interest in providing quality care and service for residents.Physical ability to perform the essential duties of the job.Provision of a satisfactory Criminal Investigation Reference for the vulnerable sector dated within the past six months.Commitment to Lakeridge Health's core values of Inclusion, Compassion, Innovation, Teamwork, and Joy.Understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices, including staff, patient, and environmental safety, and participation in regular in‑services in this area.Conditions Of EmploymentThe terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and C.U.P.E Local 6364.
