The RN Case Manager (RNCM) will report to the Primary Health Nursing Coordinator and Manager of Primary Health Care. Although the primary base of work will be in a primary care setting, some community outreach and programming will be required. The RNCM will also provide support in areas of public health and communicable disease management.
The focus of the RNCM will be to offer patient case management through holistic care coordination within a multidisciplinary health care team. The RNCM will take a lead in fostering close therapeutic relationships between the primary care customers, their families, and the wider clinical team primarily through disease management and acute conditions which involve assessment, diagnosis, referral, health teaching, advocacy, and health promotion. The RNCM will practice to full scope within the CRNS regulatory framework, including any authorized specialty practices for which they have met the required competencies and organizational supports.

Experience
- Must possess three to five years of related experience
Education
- Bachelor of Science in Nursing (BScN)
- Graduate from an approved School of Nursing
Licenses
- Advanced Cardiac Life Support Certificate (ACLS)
- Pediatric Advanced Life Support Certificate (PALS)
- Valid Class 5 driver's license
Competencies
