The Nurse Practitioner (NP) working in the Transformation Program is an integral clinical and operational leader who advances creative integrated models of care, optimize patient outcomes, improve care progression, and strengthen the connection between hospital, community, and virtual care services. Working within an interdisciplinary team, the NP delivers comprehensive assessment, diagnosis, treatment, and ongoing management for patients with complex medical needs while practicing to full legislative scope.

In addition to direct clinical care, the NP provides leadership in care progression by supporting timely clinical decision-making, facilitating discharge planning, reducing avoidable length of stay, and identifying opportunities to optimize patient flow throughout the patient’s hospital journey. The NP works collaboratively with physicians, nursing, allied health, and operational leaders to proactively address barriers to progression, ensuring patients receive the right care in the right setting at the right time.

The role extends beyond the inpatient environment to provide clinical and strategic leadership in developing and advancing innovative models of care that reduce unnecessary hospital utilization and strengthen transitions across the continuum. This includes identifying and implementing admission avoidance strategies, supporting alternatives to hospitalization where clinically appropriate, integrating virtual care into patient management, and enhancing partnerships with community providers and services to ensure seamless transitions from hospital to home or other care settings.

The NP serves as a clinical leader in advancing evidence-informed practice, quality improvement, and system transformation initiatives that improve patient outcomes, experience,



and health system efficiency. Through collaboration across acute care, ambulatory services, home and community care, primary care, and specialty programs, the NP contributes to the design and implementation of integrated models of care that promote continuity, reduce fragmentation, and enhance access for patients with complex medical conditions.

- Current Certificate of Registration with the College of Nurses of Ontario (CNO) as Extended Class (Nurse Practitioner)
- Completion of a Masters Degree in Nursing
- Minimum of 5 years recent (within 2 years) clinical experience working with older adults presenting to hospital with multi-morbidity, frailty and social care needs
- Demonstrated knowledge of the chronic disease management framework
- Demonstrated knowledge of community resources to support integrated discharge planning with community partners
- Demonstrated ability to critically appraise the literature, evaluate and implement best practice protocols
- Demonstrated knowledge in quality improvement and the use of quality improvement methods and tools
- Proven leadership abilities, including the ability to think strategically, influence decision making, affect culture and behavior
- Proven effective communication and presentation skills
- Proven success in implementing innovative, effective, value-added initiatives using change management and project management theory and frameworks
- Demonstrated ability for independent action, initiative, problem solving, organization and prioritization
- Demonstrated critical thinking, interpersonal and communication skills, both written and verbal
- Demonstrated ability to be flexible and adapt positively to constantly changing clinical situations
- Demonstrated ability to utilize information technology
