Working collaboratively and as part of a multidisciplinary team, the Social Worker II works in a fast-paced workplace to provide Social Work supports to patients and families who are receiving service within the ICU and Emergency Departments. Services include psychosocial assessment, crisis intervention, education, treatment, advocacy, resource‑finding, and consultation, making appropriate referrals both internal and external to the hospital and liaising with community agencies/programs. The Social Worker participates in non‑patient care activities such as program planning, evaluation, quality improvement research and staff development, along with other duties as required. Please note this position is on a 5 on and 5 off rotation including weekends and long weekends.Description:As a Social Worker II, you will be a member of an inter‑disciplinary team and provide a range of social work services to patients and their families.
