$90,000 - $110,000ResponsibilitiesProvide education on pharmacological therapies, promoting adherence and understanding of treatment plans.Offer evidence-based consultative advice to patients and team members on medication management.Support medication reconciliation in collaboration with local pharmacies and healthcare agencies.Manage complex medication regimens for acute and chronic conditions, (e.G. warfarin and smoking cessation therapies)Identify and mitigate potential drug interactions or compliance challenges.Collaborate with the team to develop, implement, and evaluate care plans.Act as a drug-related information resource, providing updates on guidelines, safety alerts, and pharmacotherapeutic advancements.Contribute to the development of clinical programs that enhance patient outcomes.Drive quality improvement initiatives to optimize care processes and outcomes.Maintain accurate and comprehensive documentation in the EMR system.RequirementsCurrent Pharmacist licensure (Part A) in good standing with the Ontario College of Pharmacists requiredBoard Certified Geriatric Pharmacist certification an assetAdditional credential and training with respect to dementia care an assetAt least 2 years recent clinical experience in the health care sector,
