ResponsibilitiesProvide a range of primary care services to the patients of the Centre de santé Univi Health Centre (CSUHC); perform client assessment, follow‑ups, triage, screening, and counselling.Collaborate with nurse practitioners, physicians and other healthcare professionals to develop and implement individualized care plans.Participate in the development, delivery and evaluation of educational and health promotion programs.Maintain accurate and timely records and encounter data in patient electronic medical record (EMR).Participate in chart and case review.Communicate information clearly and concisely to patients and health‑care providers on a regular basis.Educate patients and their families about healthcare, treatments, and procedures.Participate in the assessment and reassessment of patient care needs.Adhere to policies and procedures as well as professional standards.Other duties as assigned by the Program Director.QualificationsBachelor’s Degree in Nursing, or Diploma with relevant experience.Membership in valuable standing with the College of Nurses of Ontario.Work experience in a primary care setting preferred.Knowledge and ability to assess the needs of various age groups.Experience in the planning,



implementation and evaluation of various health‑related topics in a community setting.Superior problem‑solving skills with the ability to prioritize and time manage in a demanding office environment.Knowledge of community resources, especially in regards to health and social services.Ability to work within an interdisciplinary team.Current certification in Basic Cardiac Life Support (BCLS).Bilingualism is required (English and French).Solid working knowledge and ability to navigate a PC windows environment, including shared drives and Microsoft applications.Valid Ontario’s driver’s license and access to a vehicle, would be an asset.WORK CONDITIONSSalary range: $65,169.10 - $81,927.HOOPP pension plan and competitive benefits.Condensed workweek of four days.A criminal records check will be required prior to hiring.Univi Health Centre is an equal opportunity employer and welcomes applications from all qualified individuals in Ontario.
