In this role, you will provide support, guidance and referral services to community members, including individuals at risk and/or in conflict with the law. You will also play a significant part in the youth diversion program, adult and senior outreach initiatives, violent threat risk assessments and the situation table.Here’s what you’ll get to do:Review occurrence reports containing criminal charges for young offenders and reports for children, youth and families requiring additional support.Provide outreach and case management to connect individuals and families with support and/or resources.Identify acutely elevated risk situations involving children, youth or adults and complete the situation table referral form as required.Attend weekly situation table meetings and participate in interventions with community partners to mitigate risk.Act as a liaison between HRPS and the community and assist in navigating community resources.Collaborate with Community Mobilization Bureau officers to develop and execute intervention plans aimed at reducing police contact with complex, high‑need individuals.Provide support and referral services to victims of crime.Conduct restorative justice conferences.Educate officers on evidence‑based, trauma‑informed modalities and shift perspectives to reduce police contact,



balancing community safety with individual wellbeing.Support and educate district officers on appropriate options for dealing with youth offenders and at‑risk individuals.Liaise with investigating officers, crown attorneys, probation/parole and other criminal justice partners to provide advice and recommendations regarding case disposition.Assist in assessing the viability and outlining stipulations of contracts made with youth and follow‑up to ensure compliance.Represent the Service on regional and community‑based committees and contribute to community projects and initiatives.Produce key outputs that reduce calls for service for officers through addressing underlying community needs and increasing officer capacity via training and social work consultation.Maintain case files and document activities related to each case on a regular basis.Maintain statistical information to support interim and long‑range analysis of program impact and effectiveness.Maintain knowledge of community resources available within each community.Perform other related duties as assigned.What you’ll Bring to the RoleEducation, Certifications and QualificationsMinimum bachelor’s degree in Social Work (BSW).Current Registered Social Worker in good standing with the Ontario College of Social Workers and Social Service Workers.Previous Experience, Knowledge and SkillsMinimum of five years of experience as a Social Worker.Clinical experience in conducting psychosocial assessments, threat assessments, and providing mediation and strategic counseling.Experience in youth, adult and senior population settings.Understanding of Halton‑based community agencies.Positive knowledge of the Justice System,
