Personal Support Worker (Senior Services)

TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals. TNO is seeking THREE (3) compassionate and skilled Personal Support Workers (PSWs) for full-time permanent roles in our Integrated, Personal Support and Enhanced Respite Care Services program.

Enhanced Respite Care will enhance TNO's current Integrated and Personal Support services in providing accessible, culturally responsive respite care, as well as Naturally Occurring Retirement Communities (NORC) Facilitator/Connector in Thorncliffe Park, Flemingdon Park, Taylor Massey, St.

James

Town and surrounding areas. The program will increase professional in-home and community-based respite hours, enhance existing program and service delivery within the Senior portfolio, and extend service availability during days, evenings, nights and weekends.

All programs/services in the Seniors portfolio help adults 55+ age safely at home, remain connected to primary and community care, and access coordinated supports. We work alongside health care partners, allied health providers, and community agencies to ensure seniors receive timely, appropriate services while staying active, engaged, and supported in their neighbourhoods.

This role delivers essential personal support to seniors and adults with illness, restorative care, cognitive decline, physical limitations and disabilities enabling them to live safely and independently in their homes. The ideal candidate has 2–3 years of experience, holds a Personal Support Worker certificate, and has experience working with seniors and diverse low-income, multi-racial, and multilingual newcomer communities. Candidates must hold a valid CPR certification and be fully vaccinated against COVID-19.

They must also be fully inoculated and tested for Hepatitis and Tuberculosis (TB) in accordance with community health worker safety standards.



Successful candidates are required to complete a Vulnerable Sector Screening and must be able to travel within East Toronto and Downtown East; access to a vehicle is considered an asset. Fluency in a second language, particularly Hindi and/or Urdu, is preferred.

DUTIES & RESPONSIBILITIES

- Provide personal care including bathing, dressing, hygiene, and grooming.
- Support with safe mobility, transfers, and positioning.
- Provide respite care and assist with Activities of Daily Living (ADLs).
- Provide meal preparation and feeding support aligned with dietary needs.
- Offer medication reminders following program guidelines.
- Observe and report changes in client health, behaviour, or home environment.
- Home & Safety Support
- Perform light housekeeping to maintain a clean and safe environment.
- Follow all infection prevention and control (IPAC) procedures.
- Maintain a safe working environment for clients and staff at all times.
- Documentation & Confidentiality
- Accurately document all activities using AlayaCare (EMR).
- Clock in and out at the beginning and end of each client visit.
- Ensure that TNO cellphone and GPS features remain on and active during work shift.
- Ensure client confidentiality, privacy, and security following PHI guidelines.
- Complete incident reports, daily activity notes, and safety checklists as required.
- Teamwork & Coordination
- Collaborate with TNO staff across programs and external service providers.
- Participate in team meetings, ongoing training, and quality improvement initiatives.
- Communicate promptly with the Supervisor regarding client changes or concerns.

Screening – As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening. Job Type: Full-time permanent position.

Successful candidates must be available and willing to work across all catchment areas served by the program, including TNO locations and client homes in Thorncliffe Park, Flemingdon Park, Teesdale/Taylor-Massey, St. James Town, and surrounding areas.

Candidates may be assigned to different locations within the program’s catchment area based on operational needs.

Schedule: 35 hours per week on a 24/7 rotating schedule, Sunday to Saturday.

Successful candidates must be available and willing to work all shifts on a consistent rotating basis, including daytime, evening, overnight, and weekend shifts. Shift assignments will vary based on operational needs and the rotating schedule.

Vacancy Status – This posting is to fill three (3) current positions.

Rate: $25.50 per hour
