Your Opportunity:
The physiotherapist is a member of a multidisciplinary team who collaborate and participate in the provision of Primary Health Care services to patients and their families, in the South Health Campus Family Medicine Teaching Clinic. The physiotherapist will integrate therapeutic interventions and rehabilitation strategies for patients and families in the Patients Medical Home, determining, implementing and evaluating appropriate rehabilitation strategies. The Physiotherapist collaborates with the physicians, the multi-disciplinary team and patient(s) and families to enable patients to achieve optimum physical functional independence in injury recovery, pain management, chronic conditions and illness, and functional decline, implementing and evaluating appropriate rehabilitation strategies. The Physiotherapist contributes to resident education, quality improvement, research and their professional community. This position will be part of Primary Care Alberta.
Description:
As the Physiotherapist I (PT), you will provide therapeutic interventions to enable clients to achieve optimum physical function and independence in development, disease, disability, chronic conditions, illness and functional decline. You will provide intervention (i.e. prevention, triage, screening, assessment, diagnosis, goal setting, treatment, management, education, consultation, fostering resourcefulness, case management, referral and transition support to individuals, groups, populations and communities. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. The PT collaborates with patient(s)
and families in determining, implementing and evaluating appropriate rehabilitation strategies. You will participate in a variety of interprofessional teams to build awareness, enrich supports and engage in quality improvement; partnering with healthcare providers, patients, families and other stakeholders to advance services for their clients. You will contribute to quality improvement and their professional community; participates in peer mentoring; and supervises students. You may provide clinical supervision to Therapy Assistants, Therapy Aides and Provisional Physiotherapists.

Required Qualifications: Bachelor's degree in Physiotherapy from an accredited university program. Current registration with the College of Physiotherapists of Alberta.
Additional Required Qualifications:
3 to 5 years physiotherapy experience/practice. Experience working in an outpatient, community and or primary care setting. Knowledge and understanding of the Patient Medical Home. Current BLS.
Preferred Qualifications:
Experience working with and electronic medical record (specifically Med Access). Aptitude for working and performing in an environment of continuous change and learning. Experience as a preceptor for students in clinical practicums, and teaching family medicine residents.
