A Registered Practical Nurse should also promote the values, goals, and philosophy of the Home.ResponsibilitiesDevelopment and modification of the care plans in response to residents’ individualized health care needs, change in health statusLiaises with other health care personnel, agencies and familiesMaintains good relationships with team members, residents, families and general publicEngages in many meaningful interactions with residents during your entire shiftSupervises all activities on the unit, takes charge as requiredMonitors performance and completes performance evaluation for the PSW’sDirects, assigns and evaluates the work of the PSW’sMay be in charge of Home as required.



Utilizes leadership skills to identify problems and problem- solving on the unitMonitors that care is consistently delivered with the resident Bill of Rights.Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reportedRequired to participate in the improvement of the indicators and achieve satisfactory resultsActively listens to resident and family concerns and takes appropriate action to resolve the issue in accordance with the policies of the Home.Transcribes and process physician's orders for medication or other treatment in accordance with CNO standardsOrders and ensures nursing supplies and equipment are properly and efficiently usedConducts unit rounds at the beginning and end of shiftAdministers medications and treatments in compliance with the policies of Pharmacy provider, the Home and the Standard of the College of Nurses. Documents and reports all errors and omissionsCompletes and ensures residents' clinical care assessments, care plans, and documentation are accurate, and up to date in accordance with CNO standards and changes communicated to interdisciplinary team, residents, and SDM/POAs as requiredAttends and participates in resident care conferences as required.Maintains accurate and complete records of nursing observations and care in compliance with the legislative requirements and the policies of the HomeAssesses changes in health status and reports changes to Substitute Decision Maker (SDM) and interdisciplinary teamCompletes assessments (RAI-MDS and other assessments)
