The role of the Refugee & Immigrant Specialized Experience (RISE) Case Specialist (RCS) is to provide 1:1 individual/family-based culturally competent, GBA+ & trauma-informed, strengths- based 1:1 navigation & co-ordination for high-needs clients in first language, in the Surrey and Delta communities, to address their needs, strengthen personal capacity & foster independence for a period of 12-18 months until the client is ready to graduate from the program.The RCS will provide individual and/or family eligibility, ongoing and exit assessment sessions as needed in line with the Case Management Framework. The RCS will provide customized systems and services navigation and coordination support, cultural brokerage, knowledge and skills building sessions for adapting to life in Canada, targeted referrals and bridging to relevant programs and services within the community, and accompaniments to facilitate equitable access to services.Key Areas of ResponsibilityConducts outreach to solicit client referrals to the program (including working with IRCC RAP)Responds to client referrals by outlining the program eligibility and servicesConducts client intake of eligible clients, including service consent, needs assessment,



settlement planning and provides information on, and referrals to other community service providers, resources and professionals as requiredConduct needs, assets and capacities through interviewing clients, preparing case historiesPlans, prepares and conducts life skills training, encouragement and support (individual service)Participates in the development, and implementation of group services, including the workshops, courses, workshop series and support groupsProvides in-home and community support and guidance to client families; facilitates positive family communication, assists clients in making positive changes and solving problems related to integration and self-sufficiencyConnects clients to community resources and services through language and cultural supportProvides emotional support, cultural brokerage and feedback to clients including crisis response and interventionDetects issues such as depression, family violence, substance abuse, intergenerational conflicts through the early identification of warning signs that may be present in part as a result of the cultural adjustment challenges faced by the client/client family and to refer clients to the appropriate support servicesProvides crisis counselling as needed to ensure IRCC clients are able to access settlement servicesDevelops service/action plans that ensure that are integrated and holisticIn more complex cases,



participates in integrated case management meetings or meets with professionals and social workers to report clients’ activities and progress, discusses case planning, issues and concernsMaintains related client records for monthly iCARE reporting. Produces reports, including intake, progress, and exit in accordance with established policies and guidelinesLiaises with other community service providers, professionals to coordinate services and to ensure that interests of clients are considered in decisions affecting themAccompanies clients to meetings and appointments as neededAdheres to agency policies and procedures and standards of practiceQualified CandidatesBachelor’s Degree in related human/social service fieldOne (1) year recent experience in case managing vulnerable refugee populations, or an equivalent combination of education, training and experienceAbility to speak/read/write English, Dari & Pashto at a professional levelDemonstrated recent experience in conducting client needs assessments with multi-barrier vulnerable immigrant and refugee populations and implementing a holistic service or action planDemonstrated ability to implement skilled boundaries with clientsKnowledge of the refugee experience and refugee settlement issues and knowledge of community agencies, services and resources, especially in the communities of Surrey and DeltaDemonstrated ability to work independently, problem solve with confidence to ensure effective case managementWorking knowledge of standard office equipment and software applicationsA valid class 5 Drivers’ license and access to a vehicleAbility to work a flexible scheduleA Criminal Record Check is a requirement of employment.#J-18808-Ljbffr
