Overview

Reporting to the Patient Services Supervisor, the Registered Nurse (RN) is a part-time, contract position (2–3 days per week) through March 31, 2027. While based primarily within the Connect Clinic, this role provides specialized nursing services that complement Physician and Nurse Practitioner (NP) care, including RN-led clinics, population health initiatives, completion of medical forms, and chronic disease management. The RN also plays a key role in advancing organizational quality improvement, smoking cessation services, health promotion, and the development and delivery of group programming across the Peterborough Family Health Team (PFHT).

Responsibilities

- Provide comprehensive, evidence-informed nursing care to patients accessing the Connect Clinic, practicing within the full scope of the Registered Nurse role and College of Nurses of Ontario (CNO) Standards of Practice.
- Conduct comprehensive health assessments, develop nursing care plans, and provide clinical interventions for acute, episodic, preventative, and chronic health concerns.
- Perform patient triage, clinical assessments, nursing interventions, immunizations, phlebotomy, injections, wound care, and other nursing procedures as appropriate.
- Assess patients and independently complete medical forms and clinical documentation within RN scope of practice, consulting with physicians or nurse practitioners when required.
- Review, interpret, and follow up on laboratory and diagnostic results in collaboration with physicians and nurse practitioners to ensure timely continuity of care.
- Manage and monitor clinical inboxes, patient follow-up, and care coordination to support safe and efficient patient care.
- Provide patient education, health promotion,



disease prevention, and self-management support using evidence-based practice.
- Collaborate within an interdisciplinary team to coordinate care, facilitate referrals, and support smooth transitions between healthcare providers and community services.
- Maintain accurate, timely, and professional clinical documentation within the electronic medical record (EMR).
- Support the development, implementation, and evaluation of clinical workflows, policies, medical directives, and evidence-informed best practices.
- Participate in quality improvement initiatives, program evaluation, and organizational projects that enhance patient care and service delivery across the Peterborough Family Health Team.
- Contribute to the planning and delivery of PFHT group programs, health promotion initiatives, and smoking cessation services as required.
- Assist with clinic operations, including maintaining clinical supplies, vaccine inventory, infection prevention and control standards, and emergency preparedness.

Other related duties may be assigned in the interest of the efficient operations of Peterborough’s Family Health Team and the effective management of our patient populations.

Qualifications

- Completion of a Bachelor of Science in Nursing
- Minimum five years of clinical experience, preferably in community, outreach, or primary care settings




- Registered and in positive standing with the College of Nurses of Ontario (CNO)
- Active Knowledge of CNO Standards & Guidelines
- Member of Registered Nurses Association of Ontario and active knowledge of best practice guidelines
