Your Opportunity

Nurse Case Manager in the Chronic Kidney Disease Clinic in Red Deer combined with Modality Nurse Position for the Central Zone (nurse to teach patients about treatment options available for end stage kidney disease) This position is in RED DEER. Successful candidate may be required to complete aspects of their orientation/training at Kaye Edmonton clinic depending on current knowledge and skill level. Travel to Satellite Dialysis Units within Central Zone for patient modality teaching required.

A member of the multidisciplinary team provides care to patients with stages 4 and 5 Chronic Kidney Disease (CKD). The CKD RN case manager is responsible for the assessment, planning, implementation and evaluation of patient care. The RN uses nursing knowledge in addition to treatment guidelines to develop a patient care plan using a case management model.

The RN reviews lab results, symptoms and other diagnostics to assess the patient’s response to care and communicates results with the nephrologist. The RN provides education to patients including a nursing assessment of past health history, current health status, learning needs, self-care ability and values and beliefs around health care issues and treatment. The RN provides formal education for patients and families about Renal Replacement Therapy and Conservative Kidney Management.

The RN also participates in program development and quality improvement initiatives.

Description

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.

PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).

Current Basic Cardiac Life

Support - Health Care Provider (BCLS-HCP) certification.

Additional Required Qualifications

Current CRNA registration and Basic Cardiac Life Support-Health Care Provider. BScN required. Minimum of 3 years of recent clinical nephrology nursing experience required (within 1 year) Previous experience in chronic disease management and nursing case management.

Demonstrated ability in formal patient education. Excellent time management skills and ability to work independently in a quick-paced environment with high patient volumes. Demonstrated patient assessment skills on medically complex patients.

Must be able to lift 40 pounds and assist with transfer of patients. Must be able to push bariatric patients in a wheelchair. Must be able to type on a keyboard for extended periods of the workday. Must be able to wear N95 masks

Preferred Qualifications

Previous experience in the CKD clinic. Previous experience providing modality education Previous experience in AKC-North Experience with Connect Care
