This program provides $25,000 in grant funding to eligible RPNs/RNs/ Nurse Practitioners in exchange for a two-year full-time commitment. The St. Joseph's Health Care Primary Care Diabetes Support program (PCDSP) brings together comprehensive diabetes education and leading-edge care and research. Our interdisciplinary team of care providers and educators work to support our patients' goals and health care needs, providing a continuum of services to ensure optimal health and quality of life.

Serving southwestern Ontario, the Primary Care Diabetes Support Program is affiliated with Western University, the Lawson Health Research Institute and the Center for Studies in Family Medicine. Our multidisciplinary team consists of certified diabetes educators, registered nurses, registered dietitians, nurse practitioners, social service worker, administrative staff and physicians. The team is operating out of the St.

Joseph's Family Medical and Dental Centre.

The PCDSP provides diabetes care, support and education for individuals diagnosed with diabetes, who are dealing with various challenges that make managing diabetes more difficult, such as poverty, language barriers, social isolation, addictions or mental illness. The program also provides diabetes care to individuals without a family doctor.





The Registered Nurse works collaboratively with the interprofessional team to provide client-centered evidenced-based and coordinated care to patients. The RN focuses on providing assessment, screening, healthy lifestyle support, education, and chronic disease management with a goal of improving health outcomes. The successful candidate will utilize their extensive primary care and diabetes knowledge to meet the needs of the patients.

The role may also support the participation in educational programming, as well as engage in relevant research activities.

Essential Qualifications

- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Valid registration with the College of Nurses of Ontario
- Bachelor of Science in Nursing
- Certified Diabetes Educator
- Valid CPR certificate
- Minimum of 3 years recent clinical experience in primary care or diabetes education program
- Demonstrated knowledge of family practice nursing throughout an individual's life cycle, along with comprehensive assessment skills
- Knowledge of adult learning principles and experience in the development of teaching materials and the teaching of patients, families and caregivers
- Demonstrated ability to work in a professional practice environment of shared decision making with a vision for creative ideas leading to quality outcomes and excellence in care
- Superior interpersonal, coaching and communication skills to effectively work within an interprofessional team setting




- Demonstrated creative initiatives in care planning, problem-solving, conflict management, and independent handling of priorities and emergencies
- Demonstrated computer experience, particularly with Outlook, computerized patient information systems, Microsoft Office products
- Experience in relevant research activities

Preferred Qualifications

- Proficiency in French would be an asset
- Minimum three years of recent clinical experience in patient diabetes management
- Evidence of continuous learning and use of research in diabetes care nursing practice
- Demonstrated knowledge, skills and expertise in best practice patterns and translating them into clinical practice as it relates to Diabetes Canada's Clinical Practice Guidelines for the Management of Diabetes
- Knowledge and skills with a multi-system focus on physical assessment, consultative support and evaluation specific to the care of patients with diabetes
- Clinical experience providing diabetes care to marginalized populations (newcomers to Canada, individuals with English as a second language, individuals with serious mental illness, individuals without a family physician)

Immunization Requirements

- Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
- Provide documentation of the Tuberculosis skin testing
