In accordance with the established mission, vision, values, and strategic priorities of Northern Health, and under the general supervision of Manager or designate, and practicing in accordance with the standards of qualified practice and code of ethics as outlined by the BC College of Nurses and Midwives (BCCNM), this position provides enhanced quality and continuity of care for clients preparing for discharge from hospitals by providing a sound referral system to the Community Interprofessional Team (IPT). In collaboration with the interdisciplinary team, the Nurse provides community care input to the development of individualized client discharge plans and refers to the Interprofessional Team including the Long-term Care Managers, to facilitate access to Home Support, Adult Day Programs, Assisted Living and Long-term facility care.

Shift Rotation/Hours of work: Monday to Friday, Days, 09:00 to 17:00
