Our mission is to maintain a best-in-class portfolio that consistently delivers excellence in care and services while ensuring economic sustainability.Reports to:Director of CareThe MDS/RAI Coordinator supports residents by managing assessments, ensuring accurate documentation, and helping the care team provide the best possible workplace and care.This Role Will Include Duties Such AsCoordinating the completion of MDS assessment on all residents within established timelinesUnderstanding how the MDS assessment integrates with existing clinical tools and provides leadership in streamlining these processesUnderstanding and providing critical analysis of data at different levels of aggregation (Resident, Home Health Zone, etc.)Articulating for staff the strengths of MDS as a clinical assessment toolArticulating the financial implications of coding inconsistenciesActing as key contact and liaison with Health Zone/AH&W/CIHI/PCC,



etc.Developing facility specific material within the PCC program as requiredResponding to Health Zone on all submission errorsFacilitating response and correction of submission errors with facility staffCoaching key facility personnel to build capacity for understanding RUG patient classification levelsDeveloping and introducing processes and systems to improve accuracy of RAI contentFacilitating audits of RAI assessments for content accuracyParticipating in strategic planning for program development/improvementResponsible for Clinical and Admin Setup role to support MDS within PCC software programInterprets reports generated by Health Zone/AH&W or CIHI and provides critical analysis for HomeUnderstands the development of RAP’s, quality indicators, and RUG case mix productsThe Qualified Candidate Will Have3-5 years' experience in Long Term Care, experience with MDS an assetRegistered as a RPN under CNOKnowledge in nursing rehabilitation and working OT and PTStrong analytical and assessment skillsAbility to lead training and education with Adult learnersKnowledge of InterRAI suite of tools, MDS/RUG III patient classification systemsEffective communication skillsAdditional Requirements IncludeA negative vulnerable sector check completed within 6 months of start date.Completion of a 2 Step TB Skin test within the last 12 months of start date.#J-18808-Ljbffr
