Working under the direction of the Health Director, this position is responsible for delivering and supporting programs aligned with the Community Health Plan and ensuring high-quality care for individuals, families, and the broader community.This role includes both clinical and community-based nursing services, with a strong focus on prevention, health promotion, education, and chronic disease management.Key ResponsibilitiesCommunity Health & Clinical CareProvide holistic nursing care using assessment, planning, implementation, documentation, and evaluation of client careDeliver community-based health clinics and home visitsSupport prenatal, postnatal, newborn, and early childhood careProvide chronic disease follow-up in collaboration with Home and Community Care staffSupport communicable disease control activities, including tuberculosis, STI prevention, and contact tracingManage immunization programs and seasonal vaccination clinics (e.G.,
