Job DescriptionThe Social Worker will report to the Clinical Services Manager, Functional Enhancement Unit, and will be part of a multi-disciplinary team providing care to patients on the Functional Enhancement Unit.AccountabilitiesSupport the needs of the social work teamDemonstrate qualified accountability, strong teamwork skills and a willingness to participate in quality improvement projects related to the unit and the organizationQualificationsMasters degree in social work requiredThree to five years experience working with hospital or related experienceKnowledgeable in community resourcesEligible for OCSWSSW membershipKnowledge and experience working with relevant legislation (i.E., SDM Act, POA, Consent and Capacity, etc.)Knowledge of the psychosocial impact of chronic illness on the person and the familyExperience with MDS and FIM scoresExperience with post-stroke populationExcellent communication and interpersonal skillsAbility to work as part of a multi-disciplinary teamMay be required to travel between and work at all Osler sitesExcellent organization skills and ability to set priorities as requiredMust demonstrate Osler's Values of Respect, Excellence, Service, Compassion,
