Mental Health Counsellor (Part-Time, Regular)The Thames Valley Family Health Team is one of the largest Family Health Teams (FHT) in Ontario. With more than 125 physicians across 21 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive, collaborative primary health care.The Mental Health Counsellor provides assessment, ongoing treatment and management of biopsychosocial aspects of health and well‑being for identified patients. The Mental Health Counsellor participates in the development, implementation, monitoring and evaluation of programs to meet the needs of the TVFHT patient population. Working in collaboration with an inter‑professional health care provider team, the Mental Health Counsellor assists health promotion, disease prevention, and chronic disease management across a patient’s lifespan. This role is currently providing virtual care.RequirementsGraduate of an accredited universityRegistered and in good standing with a regulated health professional college (College of Nurses on Ontario or Ontario Collegeof Social Workers and Social Service Workers, College of Occupational Therapists of Ontario)Minimum of five years clinical work experienceExtensive mental health clinical training, generally acquired through the completion of post‑graduate studiesKnowledge and experience in mental health treatment and management (evidence‑based treatments such as CBT)Skills & AbilitiesAdvanced level of mental health knowledge and experience working with adults, children, youth & families,



acquired through a minimum of 3‑5 years supervised clinical practice in this areaUnderstanding of addictions care and navigationExtensive mental health clinical training, generally acquired through the completion of post‑graduate studies (MSW, OT, RN, BScN, CPMHN(C) or equivalent) and membership in a skilled collegeExcellent clinical skills and a minimum of 3 years experience assessing and managing mental health presentations commonly seen in primary careExperience related to the delivery of efficient primary care, including the ability to share information and teachExperience with developing and facilitating groupsKnowledge and proficiency in current, evidence‑based methods and practices of social work in primary care settings, with an emphasis on chronic disease management, health promotion and risk reductionAbility to demonstrate an optimistic disposition toward new experiences and change in generalMonitor and evaluate service delivery for individual clients, communities,
