Due to the nature of this role, candidates will be required to attend events in and around York Region and South Simcoe with regular travel in York Region.As a leadership team member, the Manager of Quality, Patient Safety, and Risk provides the expertise, tools, and means to promote continuous quality improvement, a culture of safety, and risk awareness at CMHA. This involves designing and managing processes and activities throughout the organization on an ongoing basis to achieve excellence. The Manager will lead the accreditation process, provide coaching to CMHA staff in the areas of risk management, patient safety, and continuous quality improvement, engage clients/families, staff, and stakeholders in continuous quality improvement, and lead organizational‑level QI projects. The Manager is data‑driven and supports the development, tracking, analysis, interpretation, and evaluation of quality, patient safety, and risk metrics while also mentoring and guiding relevant stakeholders in the use of data and evidence‑based decision making. The Manager also supports the development, implementation and evaluation of the Quality, Patient Safety and Risk operational workplans that are aligned with organizational strategic priorities, in addition to working collaboratively with relevant stakeholders to mitigate, avoid and manage risks. This role is directly supervised by the Senior Director, of Operational Excellence, Integrated Care Planning and Strategic Projects.Duties Include but are not limited to:AccreditationManage the full accreditation cycle and act as CMHA‑YRSS Accreditation CoordinatorManage the accreditation survey process and work with relevant stakeholders to implement accreditation tools and instruments (e.G.: Worklife Pulse, Patient Safety Culture Instrument, self‑assessments)Lead the interpretation of accreditation tool and instrument results,
and support other leaders in the development of action plansIdentify areas of potential non‑compliance and assist in developing remedial action to meet complianceDevelop strategies for ongoing compliance, embedding accreditation criteria into daily work so that CMHA YRSS remains in a state of ongoing accreditation preparednessInform and instruct the management team and staff on accreditation processes and requirements to fully meet the accreditation standardsCoordinate accreditation schedule and host accreditation surveyors as necessaryEnsure accreditation surveyors are provided with appropriate documents and resources as requiredQuality Improvement, Patient Safety & Risk ManagementCoach, mentor and support teams engaged in quality improvement (QI) initiatives across the organization, promoting systems thinking and the use of QI and patient safety best practicesDesign and deliver QI and patient safety education to clinical and non‑clinical interdisciplinary teams to build capacity as requiredDevelop tools, templates, and resources to support quality improvement workProvide risk management support and advice as needed regarding high‑risk situations, complex cases, and patient safety mattersContribute to the development, facilitation or successful utilization of risk management tools and frameworks such as Root Cause Analysis, Failure Mode Effect Analysis, safeguard analysis, Lean, Six Sigma, project management etc. as requiredEngage staff, management, clients, families, partnership organizations, and other stakeholders in quality improvement initiatives as appropriateDevelop staff to lead and coach others on quality improvement tools and practices to promote a quality improvement culture at CMHAProvide project management for select quality improvement projects as requiredParticipate in internal and external projects, working groups, and other quality‑related initiatives as assigned, including representing CMHA YRSS in CMHA Ontario’s Quality Leads groupSupport the VP, Clinical Operations & Director in conducting regular environmental scans for strategic planning purposesSupport expansion of the client/family voice in the mental health/addiction systemDraft policies and procedures as requiredProvide staff support to the Board level Quality & Safety Committee and the Client and Family Advisory CommitteeWork collaboratively with Health and Safety coordinator on all relevant safety plansComplete reports for stakeholders and senior leaders as requiredLead and support the corporate incident reporting processManage and analyze incident reporting data (in addition to performance metrics, self‑assessments, and patient feedback) to identify improvement opportunities and provide recommendations in alignment with the organization’s strategic planIdentify and communicate potential risks associated with incident report trends to leadership team, and support the development of mitigation strategiesRequirementsBachelor’s degree or diploma from a college or universityRegulated health professional with registration in good standing with the appropriate college preferredContinuing education courses in quality improvement (e.G. root cause analysis, client safety, client engagement, client experience, quality improvement plans, dashboard development)Formal education in incident and risk managementLean Six Sigma certification is an assetCanadian Risk Management (CRM) designation an assetFormal education in project management or PMP designation an assetPrior Work ExperienceFive years or more experience related to quality improvement, patient safety, and risk management, including experience developing quality scorecards and helping teams evaluate quality dataExperience facilitating the accreditation processExperience in incident managementExperience in clinical risk managementExperience working in a community settingExperience in project management an assetKnowledgeExtensive knowledge of the accreditation process; preferably with Accreditation CanadaThorough knowledge of the health care agenda in OntarioExtensive knowledge of the issues, concepts and practices related to continuous quality improvement, patient safety and risk management; knowledge of client/family experience toolsProficient knowledge of legislation and regulatory setting applicable to the healthcare sector, including the Excellent Care for All Act, Quality of Care Information Protection Act and Privacy Act, Mental Health Act, PHIPA, Occupational Health and Safety Act, and Ontario Human Rights CodeAdditional RequirementsA valid Canadian driver’s license, a minimum of $1,000,000 third‑party liability insuranceWorking ConditionsThis role works extensively at a desk using a computer and telephone for up to 7 hours per day in a standard open office setting.
