Salary Range
The salary range for this position is CAD $50.86 - $68.21 / hour
Detailed Overview
In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Team Leader is responsible for administering, overseeing and coordinating the Home Health Service Line and the assigned Community Access services across Fraser Health. Administers the assigned budget, monitors, reviews and revises policies and procedures, hires staff, and coordinates the work of and supervises the Clinical Nurse Educator and designated staff. Makes training/staff development decisions and conducts performance evaluations. Assists with and supports complex client intake as required.
Responsibilities
- Administers, oversees and coordinates the services provided within the Fraser Health Home Health Service Line and the assigned Community Access services. Makes decisions regarding work assignments, provides work direction to staff and responds to administrative and clinical issues throughout the assigned services.
- Supervises the Clinical Nurse Educator and other health care providers to ensure staff follow through on specific Home Health Service Line and Community Access processes.
- Administers the budget for the assigned services and is accountable for the optimal use of client care resources; identifies and implements opportunities for improved utilization of resources and recommends/implements changes to administrative processes. Keeps Manager informed of all recommendations or decisions.
- Implements and monitors the assigned operating budget; makes recommendations regarding items such as staffing, equipment, supplies. Orders equipment and supplies for the assigned areas.
- Oversees the recruitment and selection of clinical staff including making hiring decisions; conducts performance evaluations and ensures standards, protocols and procedures are being followed.
- Monitors policies,
procedures and standards of care and is accountable for their effectiveness; makes recommendations to the Manager of long term planning. Implements processes and procedures and evaluates results.
- In collaboration with the Clinical Nurse Educator, oversees the development, implementation and evaluation of staff orientations, needs assessments, the identification of staff training requirements, and in service education.
- Coordinates and/or participates in quality improvement and risk management activities; evaluates the effect of changes in practice/procedure, generates recommendations for alternative approaches, conducts safety audits, takes corrective action and recommends changes to selected practices/protocols.
- Analyzes program statistics to identify trends and issues in the provision of assigned services. Proposes, develops and implements new or modified service delivery to meet emerging service needs.
- Facilitates team building and staff development within the assigned services by acting as a clinical role model and resource for the teams; provides for smooth implementation of practice issues; resolves issues and maintains a cooperative relationship with the Clinical Nurse Educator and the assigned staff. Advises the Manager of issues and concerns.
- Promotes research based practice and staff awareness and involvement in research activities.
- Leads/participates on committees as directed to facilitate the resolution of issues and/or advocate for the goals and objectives of the assigned services across Fraser Health.
- Carries a clinical case load as required by assisting with and supporting complex client intake within the assigned services; records relevant data.
- Performs related duties as assigned.
Qualifications
Education, Training and Experience Baccalaureate degree in Nursing from an approved school of Nursing. Five (5) years of recent,
related clinical experience within community home health including a minimum of two (2) years experience in a supervisory/administrative capacity, or an equivalent combination of education, training and experience. Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Valid BC Driver''s License and access to personal vehicle for business related purposes.
Skills and Abilities
- Clinical proficiency/expertise in the designed clinical area: gerontology, geriatrics and the needs of adults living with chronic illness within a home health setting.
- Comprehensive knowledge of nursing theory and evidence based practice.
- Comprehensive knowledge of other health care disciplines and their role in health care.
- Ability to supervise and provide leadership and work direction.
- Ability to coordinate work assignments in collaboration with staff.
- Ability to monitor staff workload and client factors and adjust assignments accordingly.
- Ability to establish priorities for the daily operations of the assigned services.
- Ability to monitor the quality of care and ensure maintenance of standards for nursing practice and support staff.
- Ability to monitor, assess and address performance issues based on professional standards of practice, and to carry out and/or provide input to the Manager formal performance reviews.
- Ability to role model, coach and mentor best practices in patient care.
- Ability to identify individual or group learning priorities to meet identified learner needs, in collaboration with other clinical staff.
- Ability to support implementation of education based on changing practices e.g. legislation and technology.
- Ability to administer the budget and provide input into minor and capital equipment.
- Ability to communicate effectively both verbally and in writing.
- Ability to work independently and in an organized and self directed manner.
- Ability to problem-solve and effectively deal with conflict situations.
- Ability to operate related equipment including applicable software.
- Ability to carry a case load by handling complex client intake.
- Ability to type 40 wpm.
