Wesley seeks an Evening Intensive Case Manager to support homeless families in Halton. Contribute to community well-being through empowering and comprehensive housing support services.

In this full-time, permanent role, you will play an integral part in implementing client-centered strategies to assist individuals transitioning to reliable housing. Reporting to the Halton Housing Services Manager, your approach should reflect compassion and accountability. Required qualifications include a post-secondary degree in Human Services and experience with vulnerable populations.

Key Responsibilities:
• Provide direct support for families in housing programs
• Process intake and community referrals
• Ensure accurate documentation for service delivery




• Assist with practical life skills and crisis intervention
• Communicate effectively with housing stakeholders

Requirements:
• Degree or diploma in Human Services or relevant field
• Must be registered with OCSWSSW
• Experience in mental health and addiction support
• Availability for flexible shifts including weekends
• Valid driver’s license is beneficial
