Carlington Community Health Centre is seeking a dedicated Family Support Resource Worker (FSRW) to assist families with children aged 0–18. You will serve as a system navigator, offering vital resources such as housing, food security, and educational services. Key elements of this role include maintaining relationships with local partners to ensure effective service delivery and creating personalized action plans based on assessed needs.
Key Responsibilities:
• Provide individualized family support and needs assessments
• Manage caseloads, coordinating access to community resources
• Deliver programs enhancing family welfare from community assessments
• Connect families to essential services and resources
• Conduct home visits and maintain client engagement tracking
Requirements:
• Bachelor’s degree in Social Work or equivalent experience
• 3–5 years in direct family support services
• Experience with multicultural and vulnerable populations
• Knowledge of child development and family dynamics
• Fluent in English and French; additional languages are a plus
