Socket.dev is seeking a Clinical Coordinator to promote diabetes care excellence across ambulatory and inpatient settings. You will facilitate coordination of diabetes activity, education programs, transition planning for pediatric patients and support day-to-day clinical services.
You will lead inter-skilled teams, manage reporting for funding contracts, and contribute to quality improvement and new program designs in collaboration with community partners.
