In this full-time role, you will coordinate clinical care and case management for individuals with disabilities. Your expertise will contribute to comprehensive individual intake assessments and successful service plans, maintaining high standards. Key Responsibilities:
• Assist in case management and clinical coordination
• Support intake assessments and care planning




• Develop relationships with individuals to understand needs
• Monitor adherence to recreation plans and care goals
• Participate in clinical meetings and case conferences

Requirements:
• Ongoing LPN, RPN, or allied health education
• Standard First Aid and CPR Level C certification
• Transparent Criminal Record Check
• Valid Driver's License and access to a vehicle
• Minimum 1 year of relevant experience
