Become a Clinical Care Supervisor with CBI Home Health in Edmonton on a 12-month contract. Oversee care for individuals, ensuring high-quality support through specialized community services.
In this full-time role, you will coordinate clinical care and case management for individuals with disabilities. Your expertise will contribute to comprehensive individual intake assessments and successful service plans, maintaining high standards. You will liaise with stakeholders, monitor care plans, and ensure adherence to local clinical standards for successful treatment outcomes.
Key Responsibilities:
• Assist in case management and clinical coordination
• Support intake assessments and care planning
• Develop relationships with individuals to understand needs
• Monitor adherence to recreation plans and care goals
• Participate in clinical meetings and case conferences
Requirements:
• Ongoing LPN, RPN, or allied health education
• Standard First Aid and CPR Level C certification
• Transparent Criminal Record Check
• Valid Driver's License and access to a vehicle
• Minimum 1 year of relevant experience
