Join St. Joseph's Health Care London as a licensed Pharmacist in the Stroke/Neurological Rehabilitation Program at Parkwood Institute, London, ON. This temporary full-time role focuses on pharmaceutical care, patient interaction, and drug therapy optimization.

As an integral member of the interdisciplinary team, you will identify and resolve drug-related issues while collaborating with healthcare professionals. Your responsibilities include medication reconciliation, patient support, and enhancing the quality of life for individuals. You will also serve as a drug information resource and contribute to clinical practice guidelines and skilled education.

Key Responsibilities:
• Identify and resolve drug-related problems
• Support medication reconciliation at patient admissions




• Verify orders for the Stroke/Neurological Rehabilitation Program
• Provide drug information and formulary input
• Engage in teaching and research activities

Requirements:
• Licensed pharmacist in good standing with Ontario College
• Knowledge of safety culture in healthcare settings
• Effective communication and interpersonal skills
• Ability to balance competing priorities
• Familiarity with scientific literature for evidence-based submissions
