As a full time Nurse Practitioner, you will join an interprofessional team dedicated to enhancing patient care across diverse populations. Essential responsibilities include developing and implementing health programs, while you leverage your experience to support patients in a primary care setting. Emphasizing health promotion and risk reduction, you will play a vital role in improving community health outcomes.

Key Responsibilities:
• Deliver primary care services to individuals, families, and communities
• Participate in program development and evaluation




• Educate patients on health promotion and disease prevention
• Collaborate with interprofessional team members
• Serve vulnerable populations, including those with mental health concerns

Requirements:
• Baccalaureate of Science in Nursing and Master’s required
• Current RN(EC) registration with College of Nurses of Ontario
• Experience in Community and/or Primary Care Setting
• Knowledge of electronic medical records preferred
• Strong communication and organizational skills
