The role focuses on delivering and coordinating home nursing services after hospital discharge, with an evening shift from 15:00 to 23:00 and bi-weekly weekend rotation.
The successful candidate will perform head-to-toe assessments, wound care, IV therapy, and diabetes management, collaborating with hospital partners to align care plans with evolving client needs.
