This full time position reports directly to the Senior Manager of Primary and Clinical Care. You will facilitate the planning, implementation, and maintenance of Home and Community Care programs while ensuring quality service delivery that meets community needs. Your leadership will guide multidisciplinary teams focused on enhancing well-being.

Key Responsibilities:
• Oversee program execution and staff supervision
• Support daily operation and service delivery
• Ensure quality and safety of client care
• Develop and evaluate Home and Community Care services
• Coordinate response to community health needs

Requirements:
• Bachelor’s Degree or College Diploma with three years of relevant experience
• Strong leadership in a health care setting
• Experience in palliative care
• Proficient in Microsoft Office Suite
• Ability to work within interdisciplinary teams
