In this vital role, you will be responsible for evaluating health conditions and prescribing effective care plans for residents in long-term care. Working with the multidisciplinary team and families, you'll ensure that all residents receive optimal support tailored to their individual requirements. Your leadership will be crucial in managing palliative care, dementia management, and more while fostering education in nursing practices.
Key Responsibilities:
• Assess and diagnose residents' acute and chronic illnesses
• Collaborate with the interdisciplinary care team
• Advocate for residents’ best interests and care needs
• Provide clinical leadership on resident care issues
• Participate in resident care conferences and planning
Requirements:
• Minimum one year in geriatric or long-term care nursing
• Leadership experience considered an asset
• Active registration with the College of Nurses of Ontario
• Knowledge of relevant Ontario health acts
• Certifications in gerontology or palliative care preferred
