Join CMHA-YRSS as a full-time Case Manager for the Assertive Community Treatment Team in Vaughan, Ontario. Support clients facing mental health challenges through innovative, individualized care.

This permanent, full time position works 35 hours weekly, including potential evenings and weekends. The Case Manager will serve clients aged 16 and older, developing personalized goals and action plans to enhance their recovery and community integration. You will utilize your knowledge of mental health services to empower clients and ensure their safety, all while collaborating effectively with team members and external agencies.

Key Responsibilities:
• Maintain an active client caseload and travel extensively
• Manage client referrals to various programs




• Develop individual empowerment plans collaboratively
• Address client symptoms and provide short-term assistance
• Uphold confidentiality and client safety standards

Requirements:
• Degree or diploma in health or social services
• Full registration with OCSWSSW or similar
• One year experience in community-based mental health
• Valid G driver's license with insurance
• Bilingual skills in relevant languages are an asset
