The Mobile Crisis Intervention Team (MCIT) works collaboratively with the OPP Haliburton. This collaboration aims to reduce the number of repeat callers who call Police Services in times of crisis due to mental health and substance use issues. The Registered Practical Nurse (RPN) - MCIT provides assessment and follow up to individuals who come into contact with Police in a mental health crisis, liaising with community partners and Hospitals to ensure that clients are connected to appropriate services, diverting clients from engaging with the Police due to a mental health crisis.

Key Responsibilities
Client Supports
Working at/out of a police detachment, respond to police calls and direction to attend in crisis situations
Attend with Police Officers at police calls involving individuals who may be experiencing mental health issues or are in crisis
Provide crisis intervention services to de-escalate immediate situation
Utilize appropriate therapeutic approaches when dealing with potential aggression
Immediate assessment of the individual’s mental health needs and, assessment of risk pertaining to the client’s mental health status
Collaborate with Police, hospital, shelter, and other crisis intervention services to support the person and family to address immediate mental health needs
Work from a trauma-informed and concurrent-disorder-informed approach
Formulate and develop support/follow-up plan for individual and care providers
Provide brief case management support and services for clients in their place of residence, and in the community
Respond to requests from police for follow up and community linkages
Assess, implement, and develop evidence-based, best-practice short term interventions based on principles of Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, and Motivational Interviewing, to promote improved psychological function and recovery
Advocate and make appropriate referrals, with consent, for needed services and supports as identified in the client’s service plan
Provide mental health, developmental disorder, physical health and concurrent disorder information to individuals, care givers and other service providers
Ongoing assessment, monitoring and supporting clients working towards treatment goals.
Perform nursing duties with clients, including medication administration, long-acting injections, monitoring of medication efficacy and side-effects.
Perform mental health status assessments, where applicable and collaborate with psychiatrists and other medical practitioners
Stay current on relevant legislation, best practices, treatment approaches and basic psychopharmacology

Documentation
Ensure all client documentation is accurately recorded in a timely manner, e.g., consents, progress notes, service agreements, crisis plans, safety reports, risk assessments etc. as required by agency and program policies and procedures
Complete all required program-related data collection (e.g., statistics, outcome measures) in an accurate and timely manner
Communicate and/or submit case notes or data as required by police, for their records, with appropriate consents
Accurate and timely completion of progress notes, as per agency policy, on client record, in conjunction with data collection processes of service delivery partners, Nursing Assessments, metabolic monitoring, Medication Reconciliation and Best Possible Medication History summaries.
Complete administrative tasks e.g., time sheets, expense claims, vacation requests, training
Completion of OCAN assessments as appropriate

Other Activities
Liaise with the Hospitals, Police, community partners
Attend and participate at Situation Table
Develop and promote collaborative partnerships with other service providers, agencies, and community partners
Maintain positive and effective working partnerships with Police, hospital staff and community partners




Participate in community outreach activities through the development of collaborative partnerships and through presentations, education and visits to community agencies and services as appropriate
On an occasional rotational basis, fill in for other Nurses in other CMHA HKPR programs, during periods of absence, to ensure service continuation.

Health and Safety – Worker Responsibilities
Work in compliance with the Occupational Health and Safety Act, applicable regulations and all organizational health and safety requirements and procedures to ensure the health and safety of clients, staff/colleagues, volunteers, and students
Actively participate in workplace, departmental and job-specific health and safety orientation, education, and training

Identify any workplace or job hazards at CMHA HKPR work sites, including defects and contraventions of the Act, reporting to their direct supervisor

Client Safety
Adhere to CMHA HKPR client safety programs including program policies as well as infection control and prevention procedures. In partnership with clients, family members, and fellow staff, maintain open lines of communication to assist the organization in identifying and achieving quality outcomes for client safety. Report client safety incidents and suggested improvements to manager or delegate.

Work in compliance with Quality Assurance Measures as outlined in Reg. 299/10 of the “Services and Supports to Promote Social Inclusion of Persons with a Developmental Disability Act, 2008"

Requirements of the Position
Education and Training
Successful completion of an approved Nursing Degree
Maintains current certificate of competence as an RPN with the College of Nurses of Ontario (CNO) and accepts responsibility for professional competency as a nurse in accordance with CNO’s practice standards.
Maintain membership in good standing with RPNAO and have liability insurance.

Assets with regard to this position




Completion of a university degree or college diploma in social/health sciences or related disciplines.
Completion of a Master’s degree is preferred, e.g., Masters of Social Work
Additional Nursing certifications, such as Mental Health Nursing.
Formal education in Motivation Interviewing, Cognitive Behavioural Therapy, Dialectical Behavioural Therapy, Trauma Informed and Concurrent Disorders.

Related and Relevant Experience
Mandatory experience
Minimum of three (3) years of experience preferably clinical, experience as a RN/RPN in a hospital or community setting.
Demonstrated knowledge of the Ontario Mental Health Act, The Health Care Consent Act, social service benefits, related legislation in Ontario and community resources in the Central East Local Health Integration Network.
Demonstrated health assessment skills, sound clinical judgment and decision-making skills
Demonstrated knowledge and skills in the provision of psycho-social assessment of individuals and families

Preferred experience
Experience in Nursing related to the mental health field, including working with addictions, dual diagnosis and/or challenging behaviours.

Working Conditions
Work primarily in an office/clinic setting, Police Station and in the community, including client homes.
Work directly with individuals with a serious mental illness and/or dual diagnosis and/or addictions who are experiencing emotional and/or mental health issues and are working towards recovery, may be experiencing crisis instability and various levels of functioning, may be experiencing poverty
Primary business hours with flexible hours pending client programming needs
Transport clients in personal vehicle, as required
There is a high volume of work as well as the unpredictable work, life-threatening situations for others such as instances of suicide
