Become a vital part of Access Alliance as a Nurse Practitioner providing essential primary care services in Toronto. This full-time, permanent position prioritizes health equity for diverse immigrant populations.

Access Alliance Multicultural Health and Community Services is seeking a dedicated Nurse Practitioner to join their team-oriented Primary Care Team. In this role, you will provide comprehensive care, manage an independent caseload, and work with vulnerable populations, ensuring quality health outcomes. Your expertise in health promotion and disease prevention will be vital as you address both individual and community health concerns.

Key Responsibilities:
• Provide primary care services including assessments and treatment
• Manage an independent caseload and triage client needs




• Tailor care approaches for diverse health disparities
• Maintain accurate client health records and follow-ups
• Collaborate within an interdisciplinary health care team

Requirements:
• Masters prepared RN (EC), active registration in Ontario
• Minimum two years working as a primary care NP preferred
• Experience with newcomer populations and community health
• Strong cultural competency and interpersonal skills
• Ability to work in a collaborative environment
