The PHC Nurse promotes quality health care outcomes working within established standards to ensure patients/clients receive a high standard of primary health care. The PHC Nurse works alongside physicians, clinic staff, and other health care professionals to promote quality health care outcomes, while working with established standards to ensure patients/clients receive a high standard of primary health care. PHC Nurse initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site. The PHC Nurse works relatively independently and is expected to make decisions based on competent assessment knowledge and skills. The PHC Nurse plays a key role in quality improvement in the function of the clinic, primary health supports offered in the community and supports linkages between primary and secondary health care.ExperienceExperience in case management is an assetPrevious experience in chronic disease management would be an assetExperience in a community based,
