As part of the ACTT team, the Social Worker (BSW) will provide essential support and rehabilitation services to individuals grappling with severe mental health challenges. With a commitment to a fully integrated, multidisciplinary approach, you will assess and coordinate treatment plans effectively. Your expertise will also encompass crisis interventions and teamwork across wide-ranging healthcare disciplines.
Key Responsibilities:
• Deliver assessment and treatment services for mental health clients
• Provide case management and support for substance use issues
• Collaborate with interdisciplinary teams for holistic care
• Continuously monitor and evaluate client progress
• Maintain detailed clinical documentation in an electronic system
Requirements:
• Bachelor of Social Work and registration with a regulatory body
• Minimum 3 years experience in community mental health
• Proficient in crisis management and decision-making
• Understanding of Mental Health Act and ACTT model
• Valid Ontario Driver's License and reliable car
