Working under the Manager of Caregiver Wellness, you'll collaborate with multidisciplinary teams and engage in community outreach efforts. This crucial position addresses the complex needs of older adults, fostering a meaningful impact in mental health and geriatric care.
Key Responsibilities:
• Conduct psychosocial and geriatric assessments for clients
• Create and implement coordinated care plans
• Provide clinical support and crisis intervention services
• Assist in navigating community resources for clients
• Facilitate therapeutic group sessions and drop-in clinics
Requirements:
• Masters of Social Work or related graduate degree
• Registered Social Worker in Ontario
• 3+ years experience with older adults
• Robust skills in crisis intervention and case management
• Knowledge of mental health and addiction issues
