Duties

- Conduct thorough patient assessments, including history-taking and physical examinations for individuals of all ages, with a focus on pediatrics and senior populations
- Develop and implement personalized treatment plans, including medication management and pain control strategies
- Administer medications, vaccinations, and perform procedures such as phlebotomy with precision and adherence to safety protocols
- Utilize electronic health records systems such as Epic and Cerner to document patient encounters accurately and efficiently
- Provide case management services, coordinating care for patients with complex needs such as dementia or chronic illnesses
- Educate patients and families on health conditions, medication use, lifestyle modifications, and preventive care measures
- Collaborate with interdisciplinary teams to ensure seamless patient care in nursing homes,



assisted living facilities, hospitals, or home care settings
- Maintain up-to-date knowledge of medical terminology, laboratory procedures, and current healthcare practices

Experience

- Proven experience working with children in clinical or community settings
- Nursing home experience with a focus on elder care and dementia management preferred
- Hospital experience involving acute care procedures and patient stabilization techniques
- Background in senior care or assisted living environments demonstrating compassion for aging populations
- Skills in pain management, case management, and laboratory procedures such as phlebotomy highly desirable
- Familiarity with medical office operations and electronic health record systems like Epic or Cerner is essential
- Solid understanding of medical terminology, medication administration protocols, and home care practices
- Experience working with diverse patient populations across pediatric to geriatric age groups is advantageous
