ABOUT THE ROLE
Reporting to the Director, Client Care, the Cantonese Mental Health Worker plays an important role in supporting clients with severe and persistent mental illness from the Cantonese-speaking community through culturally responsive, client-centred intensive case management services.

Working collaboratively with clients, families, community partners, and other service providers, the Cantonese Mental Health Worker supports clients throughout their recovery journey by providing assessment, treatment, rehabilitation and support services; facilitating access to appropriate community resources and interventions; addressing barriers to mental health services, and promoting recovery, capacity building and empowerment.

WHAT YOU WILL DO

- Support clients’ recovery through the provision of intensive case management services including: client-centred assessments, treatment, rehabilitation and support services; and working with families to promote mutual support, capacity building and empowerment.
- Perform central intake functions including screening calls and other forms of referrals and determines whether the client meets eligibility criteria; performing assessments on the need for services and facilitates access to the appropriate interventions.
- Facilitate linkages between clients and community resources and collaborating with appropriate service providers or community organizations.
- Identify issues and provide advice, consultation, support and information to consumers, their families, service providers and the community on mental health and related matters.
- Address barriers that impact on the Asian communities’ ability to access mental health services.
- Participate in providing education to community service providers and other stakeholders regarding culturally responsive service.
- Maintain effective and timely documentation of client records..

Education and Experiences

- Social Work degree or equivalent.
- Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or eligibility for OCSWSSW membership.
- At least 2 years of case management experience and/or front-line experience working with vulnerable populations.

Knowledge and Skills:

- Sound clinical skills, including assessment, intervention, advocacy, psychoeducation, supportive counselling and life skills coaching.
- Sound group work and community work skills.
- An understanding of the psychosocial rehabilitation model.
- Knowledge of mental illness and medications, with the ability to assist clients with symptom management.
- Knowledge of community resources and systems.
- Excellent communication skills.
- Strong professionalism and ability to work collaboratively as part of a team.
- Demonstrated organizational and problem-solving skills.
- Competence in relevant computer applications, particularly Microsoft Office.
- Demonstrated ability to work effectively in a multicultural setting.
- Proficiency in English and Cantonese is required.
- A valid Canadian driver’s licence and access to a vehicle is an asset.
- A satisfactory Vulnerable Sector Police Record Check is required for this position

Working Hours

- Part-time, fixed-term position (0.6 FTE), ending March 31, 2027.
- The position is scheduled for 22.5 hours per week, normally worked over three (3) days per week, at 7.5 hours per day.
- Regular working hours are 9:00 a.m. to 5:00 p.m. on scheduled working days.
- Some flexibility may occasionally be required outside regular working hours, based on operational needs.




- The specific three working days will be determined based on program and client needs.
- The position involves work in both office and community settings.
