Position: Diabetes Nurse Educator– Full Time Permanent

Reports to: Executive Director

Hours of Work: 37.50 hours per week

Bridgepoint Family Health Team

The Bridgepoint Family Health Team (FHT) is located within the East Toronto sub-region. We are an interdisciplinary team comprised of physicians, a nurse practitioner, registered nurse, pharmacist, diabetes educators, social workers, medical assistants and an administrative team.

Bridgepoint FHT Diabetes Education Program Description

The registered nurse partners with a registered dietitian in our Diabetes Education Program (DEP) to offer ongoing education, support, and resources to people living with types 2 and 1 diabetes and pre-diabetes. We offer our services to patients of the Bridgepoint Family Health Team as well as community referrals, patients attending our endocrinology clinic, and to patients of our community partners. We have several satellite locations where we deliver one-on-one diabetes self-management/group education.

Successful applicants must be fully vaccinated against vaccine-preventable diseases, including COVID-19, unless they qualify for medical exemption under provincial guidelines.

Main Responsibilities:

· Plan, implement, and evaluate diabetes education programming and partnership initiatives. Educational programming may include: group teaching, individual counselling, and other initiatives to support people who are living with diabetes or pre-diabetes. Programming may extend to the patient’s family and support system.





· Develop diabetes-related care plans based upon comprehensive needs assessment.

· Develop and evaluate diabetes education resources and counselling materials.

· Work under medical directives to assist in the medical management of patients living with diabetes (this includes making recommendations to initiate, adjust, and/or discontinue diabetes oral medications or injectables).

· Apply motivational interviewing strategies to facilitate behavior change related to diabetes self-management.

· Maintain accurate, concise and confidential electronic medical record documentation.

· Participate in meetings and educational sessions with other multi-disciplinary team members.

· Ensure the organization’s policies and procedures are carried out:

a. Maintain all aspects of confidentiality (PHIPPA) relating to clients, volunteers, staff, programs, community members, and community partners.

· Perform other duties requested by the Medical and Administrative Directors

Qualifications:

· Canadian Diabetes Educator certification (or willingness to work toward) is a priority

. Current registration/membership with the Ontario College of Nurses

· Prior specialization in diabetes care is an asset

· Experience with foot care or having a foot care certification is an asset





· Experience working in a primary care setting is an asset

· Demonstrated group facilitation skills

· Able to work both autonomously and collaboratively within an inter-professional team

· Experience with quality improvement initiatives in clinical and health promotion settings

· Experience in applying evidence - informed best practices and clinical practice guidelines

· Experience working with communities and individuals who face issues such as poverty, literacy, racism, mental health, weight bias, ageism, gender bias and other barriers to accessing appropriate health care

· Experience working within a complex chronic disease framework (multiple comorbidities, polypharmacy)

· Experience with outreach and community-based health care for marginalized populations and willing to work in multiple locations

· Knowledge of and working experience with computer systems and applications
