Registered Practical Nurses (RPN) – Primary Care Clinic

Job Type: Permanent Full time, Permanent Part time, Casual Relief

Location: Southmount Family Practice, 35 Upper Centennial Parkway, Stoney Creek, ON

Reports to: Clinic Manager

We are a community-based primary care clinic providing comprehensive, patient-centred care across the lifespan. Our team includes family physicians, nurses, allied health professionals, and administrative staff.

We focus on timely access, prevention and chronic disease management, and a welcoming experience for every patient.

Position Summary

As one of several Registered Practical Nurses (RPN) in our primary care clinic, you will support physicians and the interprofessional team by providing nursing care, performing clinical procedures, contributing to preventive care and chronic disease management, and coordinating follow-up to help patients navigate their care. The ideal candidate is organized, calm under pressure, and committed to respectful, culturally safe, trauma-informed care.

Key Responsibilities

- Performs nursing assessments (e.g., vitals, focused assessments) and documents findings accurately in the EMR.
- Provides in-clinic general nursing care for scheduled and walk-in visits as appropriate (rooming patients, medication reconciliation, history updates).
- Administers injections/immunizations.
- Works in collaboration with physicians to complete well baby visits, prenatal and annual physical assessments (including HPV screening).
- Assists with minor procedures (e.g., wound care, dressings, suture/staple removal, ear lavage, wart removal) and maintains procedure trays/supplies.
- Prepares equipment and ensures that examination rooms are clean and stocked with required supplies and materials.
- Triages patient calls,



supporting patient callbacks related to lab requests and results, test results and prescription requests using established protocols; escalating concerns as required.
- Maintains vaccine cold chain, infection prevention and control (IPAC) practices, and clinic safety standards.
- Contributes to quality improvement initiatives
- Supports a respectful environment by collaborating with colleagues and communicating professionally with patients and families.
- Maintains patient confidentiality.
- Performs other related duties within the clinic as required.

Required Qualifications

- Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN).
- Demonstrated competence in primary care nursing skills (e.g., immunizations, injections, wound care, phlebotomy/specimen collection, point-of-care testing) or willingness to be trained.
- Experience using an electronic medical record (EMR) and robust documentation practices. Preference for experience with PS Suite EMR.
- Knowledge of infection prevention and control (IPAC) standards and routine practices.
- Robust interpersonal communication and time management skills and the ability to prioritize tasks in a busy clinic setting.
- Ability to work independently and collaboratively within an interprofessional team.

Preferred Assets

- 1–3 years’ experience in primary care, family practice, community health, or an ambulatory clinic.




- Experience with immunization programs (including cold chain monitoring).
- Experience supporting chronic disease programs (e.g., diabetes education).
- Familiarity with Ontario health system navigation, community resources, and referral pathways.
- Additional certification/training (e.g., foot care, smoking cessation, wound care, phlebotomy).

Schedule & Work Environment

- Hours: Monday–Friday, daytime; occasional evening/weekend clinics.
- Start date: September 8 th 2026.
- This role is in-clinic and involves standing, bending, and assisting with clinical procedures throughout the day.

Compensation & Benefits

- Hourly wage rate: $ 26.73 - $28.96 (commensurate with experience).
