Serving 20,573 annually and providing 1,856 units of supportive housing last year, LOFT is one of Ontario’s largest mental health service providers of its kind. LOFT is also recognized as a Nonprofit Employer of Choice Award recipient.About the ProgramAssisted Living- Dunn is a 24/7 program that provides specialized assisted living supports to vulnerable older adults and seniors with complex physical health challenges, mental health challenges, addiction or cognitive impairment. Assisted Living - Dunn responds to resident’s needs so they can live safely and in valuable health in the community as they age. Through our care, we aim to postpone for as long as possible, or eliminate, any need for institutional care. The program serves 70 clients living at program sites, as well as an additional 28 clients through Hub and Spoke services in the wider community. The program supports clients living independently in the community with support and dignity. PSW, Case Manager and Life enrichment supports are provided to assist clients at home and in the community.What You Will DoReview client background information and interview clients to obtain case history and prepare intake reportsAssess clients’ skills, strengths,



and support needsAssist clients in exploring options and developing action plans while providing ongoing support and assistanceAssess and investigate eligibility for social benefitsRefer clients to social services and community resources, including legal, medical, financial, housing, employment, transportation, and daycare supportsCounsel clients living in group homes and halfway houses and assist with pre-release and release planningParticipate in the selection and admission of clients into appropriate programsImplement life skills workshops, substance abuse treatment programs, behavior management programs, life enrichment programs, senior services programs, and other community/social service initiativesMeet with clients to assess progress, provide support, and discuss challenges or barriersAssist in evaluating treatment program effectiveness by tracking clients’ behavioral changes and responses to interventionsAdvise and support recipients of social assistance and pensionsProvide crisis intervention and emergency shelter servicesImplement and coordinate the delivery of specialized community servicesMaintain communication with social service agencies and health care providers to support client progress and continuity of careMaintain program statistics and records for evaluation, reporting,
